By booking a session or submitting an intake form, you consent to the collection and use of your information as described in this policy.
2. What Information I Collect
I collect the following personal information:

∙ Date of birth
∙ Health history and session notes
∙ Payment details
This information is collected for the purpose of providing safe, effective, and personalised kinesiology sessions.
3. How I Collect Your Information
Your information is collected via:
∙ Online intake form (JotForm)
∙ Booking platform (Acuity Scheduling)
I only collect information that is necessary to provide my services to you.
4. How I Use Your Information
Your personal information is used to:
∙ Prepare for and deliver your kinesiology sessions
∙ Maintain accurate and confidential session records
∙ Contact you regarding appointments, cancellations, or follow-ups
∙ Process payments securely
∙ Comply with any legal or professional obligations
I will never use your information for unrelated purposes or share it with third parties for marketing without your explicit consent.
5. Third-Party Tools
To deliver my services I use the following third-party platforms, each of which has its own privacy policy:
∙ Acuity Scheduling — used for appointment bookings and payment processing (squareup.com/legal/privacy)
∙ JotForm — used for online intake forms (jotform.com/privacy)
∙ Squarespace — used to host this website (squarespace.com/privacy)
I take reasonable steps to ensure these providers handle your data securely, however I encourage you to review their privacy policies directly.
6. Health Information
Your health history and session notes are considered sensitive information under the Privacy Act. This information is collected solely to support your care and is kept strictly confidential. It will not be disclosed to any third party without your consent, except where required or authorised by law (for example, in cases of risk to life).
7. How I Store Your Information
Your information is stored securely within Acuity Scheduling and JotForm. Paper records, if any, are stored in a secure location accessible only to me. I take all reasonable steps to protect your information from misuse, loss, unauthorised access, or disclosure.
8. How Long I Keep Your Information
In accordance with Victorian health records guidelines, I retain client records for a minimum of 7 years from the date of your last appointment (or until a minor client turns 25, whichever is later). After this period, records are securely deleted or destroyed.
9. Accessing or Correcting Your Information
You have the right to request access to the personal information I hold about you, and to ask for corrections if any information is inaccurate or out of date. To make a request please contact me directly:

10. Complaints
